Our 'cathedrals of medicine' surrounded by their lofty moats, and populated by silo driven management, are detached not only from the constituents they ostensibly serve, but as you correctly point out, the mission of the enterprise they presumably 'steward'.

Because it's what's most comfortable to them personally. Many don't just tolerate such a culture ... they often actively feed it - using whatever means necessary to keep their chosen status quo.
